• Membership dues are $35 per year (July 1 – June 30).
• If you join (i.e., if you are completing this form) between July 1 and March 31, you will be sent all the back issues of the Chronicle for this membership year. You will be sent a dues notice for next year in early May.
• If you join between April 1 and June 30, the dues will apply to the next membership year. Your access to the website will be granted immediately, but your Chronicle subscription will begin with the August issue.
• USA Life Memberships $1,000.00 (NOTE: International Life Memberships are available at $1,000.00, plus an annual additional charge for international airmail service. This cost is currently $16/year, but will continue to be billed at our actual cost if and when the United States Postal Service (USPS) increases their rates.). The $1,000 fee can be paid over 6 months with three payments: $333, $333 and $334.

INTERNATIONAL: A $16 per year surcharge is added to the dues for members whose addresses are outside the USA to cover additional mailing costs. You may choose to waive the receipt of hard copies of the Chronicle and the Chatter and save the $16 yearly shipping fee. The Web Chronicle appears in the members’ area of the Society website about 30 days after publication.

• For payment by PayPal, once you log in for the first time, you will be redirected to the PayPal website where you can process your payment. When you are done, you will be brought back to our login page.
• For payment by check, once you log in for the first time, a form will appear with all your information already filled out. Please print it out and mail the form in with your check.
• You may retry your PayPal payment or re-print your pay-by-check form from your Member Settings page after login should you need to.
